Case (M/86)

CC : Lt. neck mass, Syncope
PI : 2018년 11월 좌측 갑상선 결절 및 좌측 경부 종물로 본원 내분비 내원하신 분으로 US FNA 상 Thyroid cancer /c neck metastasis 의심되어 수술적 치료 의뢰됨.
2019년 1월 31일, 2월 2일 syncope 증상으로 ER 경유하여 내과 입원. 입원 후 CT angio perfusion 및 심초음파 상 특이소견 없음.

Lt. carotid artery 근처 mass effect로 인한 vagal nerve compression이 원인일 가능성이 많음.
2019년 2월 8일 Total thyroidectomy /c Both CCND /c Lt. SND (level : IIa~V) 수술 시행받음.

술전 FNA 소견

•   Thyroid gland, left lobe, fine needle aspiration:

- Satisfactory for evaluation.
- MALIGNANCY.
- Carcinoma

•   Lymph node, level IV, left, fine needle aspiration:

- Satisfactory for evaluation.
- Consistent with metastatic carcinoma.

수술 소견

IJV을 보존하지 못하였고 IJV과 subclavian vein이 만나는 부위까지 cancer가 침범하고 있어 완전히 제거하지 못하고 일부 남겨둠
(혈관외과 교수와 상의한 결과 나이와 갑상선암 임을 고려함)

조직검사 결과

•   Thyroid gland, left lobe, hemithyroidectomy:

- Anaplastic thyroid carcinoma, with
1) a component of papillary thyroid carcinoma
2) squamous differentiation
3) extrathyroidal extension
4) clear but close resection margin

•   Thyroid gland, right lobe, hemithyroidectomy:

- Papillary microcarcinoma, 3mm
- Adenomatous hyperplasia.

• Lymph node, neck, level VI, left, dissection : No tumor (0/1).
• Lymph node, neck, level VI, right, dissection : Carcinoma, metastatic (1/1).
• Lymph node, neck, level IIA, left, dissection : No tumor (0/1).
• Lymph node, neck, level IIB, left, dissection : No tumor (0/4).
• Lymph node, neck, level III, left, dissection : Carcinoma, metastatic (4/11).
• Lymph node, neck, level IV, left, dissection : No tumor (0/3).
• Lymph node, neck, level V, left, dissection : No tumor (0/1).
• Blood vessel, external jugular vein, left, excision : No tumor.
• Blood vessel, internal jugular vein, left, excision : Carcinoma, metastatic (4/5).
